注意
事項 |
1. 領獎期間自97年10月06日起至98年01月05日止。領獎時，請中
獎人填妥領獎收據並在收據上黏貼0.4%印花稅票(中五獎以上者)，
攜帶國民身分證及中獎統一發票收執聯兌領獎金。中特獎 / 頭獎 /
二獎 / 三獎者請向各直轄市及各縣 / 市經指定之郵局領取獎金；中
四獎 / 五獎 / 六獎者請向各地郵局兌領。
2. 統一發票收執聯未依規定載明金額者，不得領獎。
3. 統一發票買受人為政府機關 / 公營事業 / 公立學校 / 部隊及營業人
者，不得領獎。
4. 中四獎以上者，依規定應由發獎單位扣繳20%所得稅款。
5. 中獎之統一發票，每張按其最高中獎獎別限領1個獎金。
6. 其他有關領獎事項均依統一發票給獎辦法規定辦理。

Regulations
for Prize
Money Claims |
1. In order to receive the prize money, a winner must fill out
the form on the back of the uniform invoice and present this
with his or her ID card at any post office from 2008/10/06
to 2009/01/05. A winner need not collect the prize money
in person. Such person as is entrusted by a winner to collect
his or her prize money should present his or her own ID card
along with the winning person's ID card and the winning
uniform invoice at the post office in order to receive the
prize money, or bank in order to receive the prize money.
2. If the uniform invoice receipt dose not indicate the amount
of the sale, the winner is not qualified to collect the prize
money.
3. If the buyer shown on the uniform invoice receipt is a
government agency, stat-run enterprise, public school,
military unit or a business entity, the winner is not qualified
to collect the prize money.
4. Each invoice may win only one prize.
5. For winners of the fourth, third, second, first, or grand prize,
20% withholding tax is levied on the prize.
